You may enroll your child in a Madison school at any time throughout the year. We encourage you to visit our schools to determine which one is right for your child. Tours are regularly scheduled at each school for prospective parents. Please contact the school’s front office to schedule a tour!
New Open Enrollment Dates
It is a pleasure for the Madison School District to announce the Open Enrollment dates for the 2018-2019 school year. December 1, 2017 through January 26, 2018 parents/guardians may submit an application for open enrollment to the school where you wish your child(ren) to attend. Open enrollment applications are available in the “Open Enrollment” section below. A separate application is required for each student. Open enrollment applications will be approved/denied on a “first come, first served” basis. The school office will time-stamp applications as they are received.
New Open Enrollment Students: Please submit your open enrollment application between December 1, 2017 and January 26, 2018 to receive notification of approval/denial of open enrollment prior to March 1, 2018. Any open enrollment applications received after January 26, 2018 may not be approved/denied until August, 2018. Approval for open enrollment will be based on physical capacity, current attendance, discipline records, and current academic progress.
Current Open Enrollment Students: For families who are currently open enrolled and wish to continue open enrollment at the same school, you do not need to submit a new open enrollment application. If you would like your child(ren) to open enroll to a different school for next year, you must submit a new open enrollment application between December 1, 2017 and January 26, 2018 for the school you wish your child(ren) to attend. Any open enrollment applications received after January 26, 2018 may not be approved/denied until August, 2018.

Madison does not provide bus transportation for open enrollment students. If you have specific questions about a school you are considering for open enrollment, please contact the administration of that school.

New Students Joining Madison
To enroll your child in Madison, please print, complete, and submit the following forms to the Madison School you would like your child to attend:

- If your family lives outside the school or district boundaries you will need to submit an Open Enrollment Application; please see the “Open Enrollment” section below for more information.
- State of Arizona – PHLOTE / Estado de Arizona – PHLOTE
- Student Services Questionnaire
- Native American Program Information & Forms (if applicable)
At the time of enrollment, you will also need to provide:
- A copy of your child’s birth certificate
- Immunization records
- Proof of residency (lease agreement or primary utility such as electric or water bill)

Upon acceptance of your application, the school will provide you with a login to create your PowerSchool Parent Account, where you will complete the application process and be able to update your information as needed.
For students transferring from another school, Madison will request records from the previous school once registration is completed.
Students must meet state age and health requirements. Arizona law requires that students must provide verification that immunizations have been completed before starting school. Each student’s immunization records remain in the school system once enrolled. Each time the child receives another immunization, the health file should be updated. Open Enrollment applications for the 2017-2018 school year are now being accepted. Applications for the 2018-2019 school year will begin being accepted December 1, 2017.
If your family lives outside the boundaries of your neighborhood Madison school or the Madison district’s boundaries, you will need to submit an Open Enrollment Application. Read more about our Open Enrollment procedures (updated 11/2017).
While bus transportation is typically not provided to families living outside the school’s boundaries, schools located in high traffic areas may require Open Enrollment families to utilize Madison’s bus service as a condition of enrollment.

For admission to kindergarten, children must be five (5) years of age prior to September 1 of the current school year.
The Madison School District will not offer early entry testing or admit students who are not yet five (5) prior to September 1 for the school year. (This statement is in the policy regulation, JEB-R.)
Children may be admitted to first grade who are six (6) years of age, or shall be deemed six (6) years of age if they reach such age prior to September 1 of the current school year.

Annual Registration for Returning Families
If your child is returning for the new school year, please log into your PowerSchool Parent Account, click on the “E-Registration” tab and update your information as needed. If you do not have an account, please contact your school’s front office.
