Enrollment Information

Thank you for your interest in Madison!

You may enroll your child in a Madison school at any time throughout the year. We encourage you to visit our schools to determine which one is right for your child. Tours are regularly scheduled at each school for prospective parents. Please contact the school’s front office to schedule a tour!

New Students Joining Madison

To enroll your child in Madison, please print, complete, and submit the Pre-Registration Packet  (see list of enrollment documents below) to the Madison school you would like your child to attend. Packets are also available at the school site. At the time of enrollment, you will also need to provide:

  • A copy of your child’s birth certificate
  • Immunization records
  • Proof of residency (see residency forms included in the Pre-Registration packet for more information).

Individual schools may require additional forms and information at the time of enrollment. For additional information regarding enrollment for kindergarten, please see the “Kindergarten Enrollment” section below.

Upon submission of your Pre-Registration paperwork, the school will provide you with a login to create your PowerSchool Parent Account, where you will complete the registration process and be able to update your information as needed.

For students that are transferring from another school, Madison will request records from the previous school once registration is completed.

Students must meet state age and health requirements. Arizona law requires that students must provide verification that immunizations have been completed before starting school. Each student’s immunization records remain in the school system once enrolled. Each time the child receives another immunization, the health file should be updated. Open Enrollment

Open Enrollment applications for the 2017-2018 school year are now being accepted. Forms are available in the “Enrollment Forms” section below and at each school site.

If your family lives outside the boundaries of your neighborhood Madison school or the Madison district’s boundaries, you will also need to complete an Open Enrollment Form annually. Read more about our Open Enrollment procedures (updated 1/2017).

While bus transportation is typically not provided to families living outside the school’s boundaries, schools located in high traffic areas may require Open Enrollment families to utilize Madison’s bus service as a condition of enrollment.

Kindergarten Enrollment

For admission to kindergarten, children must be five (5) years of age prior to September 1 of the current school year.

The Madison School District will not offer early entry testing or admit students who are not yet five (5) prior to September 1 for the school year. (This statement is in the policy regulation, JEB-R.)

Children may be admitted to first grade who are six (6) years of age, or shall be deemed six (6) years of age if they reach such age prior to September 1 of the current school year.

McKinney-Vento Homeless Education Assistance Improvements Act of 2001

Preschool-aged and school-aged children have certain rights or protections under the McKinney-Vento Homeless Education Assistance Act. Learn more about McKinney-Vento.

Annual Registration for Returning Families

If your child is returning for the new school year, please log into your PowerSchool Parent Account, click on the “E-Registration” tab and update your information as needed. If you do not have an account, please visit your school’s front office, and they can assist you.

If you are a returning Open Enrollment family, please make sure you have submitted your Open Enrollment form for the new school year to your child’s school. Individual schools may also require additional forms and information at the time of registration.
