Please note, new student registration will be held at the Madison District Office (5601 N. 16th Street, Phoenix, AZ 85016) June 5 – June 29 from 8:00 a.m. – 4:30 p.m. Forms will be available at this time and at each school site subsequently.
At the time of enrollment, you will also need to provide:
- A copy of your child’s birth certificate

Additional forms and information may be requested subsequently.
For additional information regarding enrollment for kindergarten, please see the “Kindergarten Enrollment” section below.
Upon receipt of your application, the school will provide you with a login to create your PowerSchool Parent Account, where you will complete the application process and be able to update your information as needed.
For students transferring from another school, Madison will request records from the previous school once registration is completed.
Students must meet state age and health requirements. Arizona law requires that students must provide verification that immunizations have been completed before starting school. Each student’s immunization records remain in the school system once enrolled. Each time the child receives another immunization, the health file should be updated. View the state’s Immunization Guide for more information.
Open Enrollment applications for the 2017-2018 school year are now being accepted.
If your family lives outside the boundaries of your neighborhood Madison school or the Madison district’s boundaries, you will also need to complete an Open Enrollment Form annually. Read more about our Open Enrollment procedures (updated 1/2017).
While bus transportation is typically not provided to families living outside the school’s boundaries, schools located in high traffic areas may require Open Enrollment families to utilize Madison’s bus service as a condition of enrollment.
